OP 2 modulates OP 1 (the audible waveform / Carrier)
OP 3 modulates OP 1
OP 4 modulates OP 1
You've got three modulators each affecting OP 1. The modulators aren't affecting other modulators.
In example 2:
OP 4 modulates OP 3 modulates OP 2 modulates OP 1 (the carrier). The modulators are modulating the next one down the line, until finally getting to OP1.
If the four blocks were in a pattern like this:

There would be no FM synthesis occurring, as there is no modulator.
